Specific Services

 

The goal of the Family Reunification Program is to assist the child in returning home to a safe and healthy family.  Each family receives guidance from a Team Leader and a Family Support Worker who: 

  • Coordinate monthly Family Team Meetings
  • Provide supported referrals to community resources
  • Establish a personalized family transition plan
  • Provide clinical and educational support through the Strengthening Families Program
  • Offer 24 hour emergency support as needed
  • Advocate for child and family safety in coordination with DHHS.

To ensure that the needs of the families are met in the least restrictive manner, services are: 

  • Team delivered
  • Available in client homes
  • Short term (up to 4 months with a possible 2 month extension upon DHHS approval)
  • Provided by licensed social workers
  • With adult family member participation
  • Based on family strengths.

During the transition period, HCI continues to work with DHHS and the court system to ensure that safety needs of the child and family are being met.  Hours and frequency of service vary according to need and position in the reunification process.
